What do I need to bring to my appointment?

For every appointment you should bring a photo ID (drivers license, school ID, etc.) and your insurance card.
At least once a year PDCHC must verify your income, residency and household size.
For this you must bring some form of photo ID, this can be used for verification of identity as well as residency. A current utility bill, letter from social worker, case worker or shelter director can also act as proof of residency. For verification of income you must bring either your most recent payroll stub, W-2, social security or disability check. For a more detailed list please download the document above.

I don't have insurance, can I still be seen?

Yes! If you don't have insurance you can still be seen. PDCHC has a sliding scale payment program for those without insurance. Just bring proof of income (W2, disability or social security check, etc.) with you to your appointment and we'll get you all sorted out.

What types of insurance do you accept?

PDCHC accepts all KY Medicaid and major commercial insurances. In some cases you must switch your PCP (primary care provider) to Park DuValle. If you have any questions please call 502-774-4401 Ext. 1230

I need a copy of my medical records, how do I get that?

To get a copy of your medical records you must first fill out and return a Release of Protected Information form. This form can be obtained above or at any PDCHC location.

I need a ride to my appointment, how do I arrange that?

Transportation eligibility is determined on a case by case basis. If you are in need of transportation to your appointment please let a member of our staff know when making your appointment.
If you need to arrange transportation and you have already set your appointment, please call 502-774-4401 Ext. 1135 or 1136. Our transportation team needs at least 48hrs notice to ensure all patients arrive before their appointment time. Transportation is available to anyone with a medical, dental or behavioral health appointment.

I have a problem with my bill, who should I talk to?

If you believe you have a problem with your bill or would like to speak to someone regarding your bill, call 502-774-4401 Ext. 1230 for assistance.
