Park DuValle is an independent not-for-profit community health center and has been serving the health care needs of residents of Louisville for more than 35 years.

Park DuValle values you as a patient, and protection of your privacy is very important to us.  In conducting our business, we will create and maintain records that contain protected health information about you and the health care provided to you as a patient under our care.

 “Protected health information” or “PHI” is information about you, including information about where you live, that can reasonably be used to identify you and that relates to your past, present or future physical or mental health or condition, the provision of health care to you or the payment for that care.

We are required by certain federal and state laws to maintain the privacy of your protected health information.
